ROLE
The role involves conducting thorough in-home evaluations to assess patients' needs and preferences. You will coordinate care options tailored to each individual's requirements and adjust care plans as their needs evolve. The goal is to help clients and their families enjoy an enhanced lifestyle by offering services that ensure they remain safe and comfortable in the familiarity of their own homes.

We Offer Flexible Care Options To Meet The Diverse Needs Of Our Clients:
Minimum if 4-hour visits
8-hour shifts
12-hour shifts
24-hour shifts
Overnight care
Our team is committed to providing around-the-clock support to ensure comfort, safety, and personalized care in the home.
Key Responsibilities
Conduct in-depth home evaluations
Develop and coordinate personalized care plans
Adjust care as patient needs change
Provide compassionate, high-quality care aimed at maintaining patient independence and well-being
Our Perfect Candidate Requirements
CNA/GNA/PC license
Strong interpersonal skills
Ability to adjust care plans to meet evolving patient needs
RESULTS
CNA certification
Complete and document all assigned tasks for each client
Report any abnormal signs from clients
Report abuse
Attend all staff meeting
Provide care in a nurturing and respectful manner.
Ability to keep CPR and First Aid Certification current.
Ability to report and record any and all incidents promptly.
Have good attendance
